All of the popular practice management systems available today are based on this outdated philosophy. This places an administrative burden on the doctors. In addition to writing medical records, they are responsible for making sure all charges are accurately recorded on the invoice, for deciding which instructions are sent to the client, and for dictating or typing referral letters. In other words, the doctors must do their job and more.

What you enter is automatically stored in the “SOAP” locations of your choice: History, Physical Examination, Plan, Medications, and Discharge Instructions are just a few examples.
